Rates
Individuals: $90 for a 50-minute session
Couples: $110 for a 50-minute session

Insurance
If you are part of the Blue Cross Blue Shield of Illinois PPO, I will bill insurance directly and then bill you for your share of the remaining balance.

Other insurance carriers may reimburse you in part for my services, depending on the benefit plan, but payment will be your responsibility at the time of service. I will provide you with an itemized statement of services so you can apply for out-of-network benefits.

Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is authorization required from my insurance carrier?
• Do I have a health care savings account (HSA) or flexible spending account (FSA) that could help pay for some costs?

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
I accept cash, checks, and major credit cards for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
